you can find it easy enough, I did.

Thanks, Paul.  I found the one we're looking for. 

http://www.ct.gov/dep/lib/dep/outdoor_recreation/scorp/SCORP_AppendixA.pdf

Be sure to use the "Other" line in each area and put in "Off Highway Vehicle" for the item.  When you get done, follow the instructions for sending it to the DEP.  It looks like you will have to print and mail it in.  I really hope some of us here will take the time to do it and mail it in.
